A recommendation isn’t very helpful if no one understands how to use it. Education is a big part of how we work. We help individuals and their supports understand the “why,” learn practical strategies, and carry them into everyday routines.

You won’t be left wondering what happened after you made the referral. We communicate with the appropriate members of the team and provide practical recommendations and documentation when indicated.
Ways we can work together
Skilled In-Home Outpatient Occupational Therapy
For individuals who need occupational therapy evaluation and treatment in the place where everyday life actually happens. We address the activities, routines, and challenges that matter at home while helping clients work toward greater safety, independence, and confidence.
Occupational Therapy Therapeutic Consultation
For individuals receiving Virginia DD Waiver services who may benefit from OT consultation related to everyday activities, routines, safety, adaptive strategies, equipment, or participation. We work collaboratively with the individual and their support team to develop practical strategies that can be carried into daily life. Learn about OT therapeutic consultations here.
Pre-Discharge Home Assessments
Sometimes you know what your patient can do in therapy, but you don't know what they're going home to. We can assess the home before discharge and provide the family—and, when authorized, the rehabilitation team—with practical information and recommendations to help prepare for the transition home.
Home & Functional Assessments
For families and professionals who need a clearer picture of how someone is functioning, what the home environment looks like, and what supports or changes may help. Assessments may address safety, accessibility, equipment, daily activities, caregiver needs, and home modifications.

Knowing someone’s diagnosis is important, but it doesn’t always tell you how that person is managing everyday life. We look at the whole picture—what the person needs and wants to do, the environment they’re doing it in, and the support available to them.

A recommendation can look great on paper and still not work for the person, family, caregiver, or environment. We consider what is practical, realistic, and sustainable—not just what is technically possible.

FHT welcomes referrals and inquiries from healthcare providers, Support Coordinators, case managers, senior care professionals, residential and day-support providers, home health agencies, rehabilitation teams, families, and other community partners. Because our services have different referral and documentation requirements, the process may vary depending on what your client needs.
